OSTEOSYNTHESIS PERFORMED FOR A KUWAITI PATIENT WITH A LEFT TIBIAL PLATEAU FRACTURE

Recently, AIH has admitted patient H.A (72 years old, Kuwaiti) who had involved in a traffic accident. The patient initially received emergency care at Vungtau’s hospital and was diagnosed with complex fractures of the left tibial plateau and the left fibula head. Due to the complexity of the injury, Mr. H.A was transferred to American International Hospital (AIH) for treatment, with the support from Kuwait Consulate General in Vietnam.

After receiving the case, the doctors from the Orthopedics and Traumatology Department performed medical examination and ordered various tests like X-Ray, CT scan of the bone, other necessary laboratory tests, etc., due to patient’s advanced age and multiple underlying conditions. MD. MSc. Nguyen Hong Trung – Medical Director and Orthopedic Traumatologist in American International Hospital (AIH), who directly performed the surgery, shared: “The patient has complex multi-fragmented fractures with depression of the left tibial plateau surface and fracture of the left fibular head, classified as grade III b according to Schatzker. After a consultation, the doctors decided to perform osteosynthesis for the patient, using screws and plates.

After more than 1 hour, the surgical team successfully completed the operation. Following the observation in the recovery room, the patient received post-op care and treatment in the Surgery Department, and early physical therapy to prevent joint stiffness and muscle atrophy, promoting bone healing process.




According to Dr. Nguyen Hong Trung, tibial plateau fracture is an intraarticular injury which is extremely dangerous and can lead to compartment syndrome. If the treatment is not timely and effective, it can impact mobility function and leave multiple lasting complications, such as knee joint degeneration, joint stiffness, which can significantly impact patient’s quality of life.

Bone fixation with plates and screws is a technique stabilizing the fractured bone ends, facilitating bone healing process with specialized materials. This method is indicated for fractures at bone ends, joint-involved fractures, comminuted multi-level fractures in the bone shaft where intramedullary nailing with transverse locking is not suitable. In the past, doctors would have to perform open surgery for bone fixation with screws and plates. However, nowadays, doctors can use the technique of bridge plating and internal splint with locks for fractures to prevent tissue damage. The advantages of this technique are outstanding, including only a small incision made to approach the fracture site via endoscopy, locked plate fixation that does not require removal surgery, materials that is durable and aesthetic pleasing, providing an excellent bone healing potential.

During the time of hospitalization, Mr. H.A shared: “I have traveled to Vietnam many times, unfortunately, this time I was involved in a traffic accident in Vungtau. When the accident happened, I was very worried because I’m here by myself, but after being transferred to American International Hospital (AIH), I have felt much reassured and very satisfied. The doctors and nurses here at AIH are really dedicated, highly skilled, and provide me with attentive care services. Following the operation, I was supported with physiotherapy according to the plan, and I also don’t experience much pain, which is very helpful for my recovery.”

On day 9th after the operation, the patient recovered well and was pain-free. He was able to walk with the crutches without the help from the medical staff.
