PRETERM BIRTH AND WHAT PREGNANT WOMEN NEED TO KNOW

According to Specialist level 1, Dr. Tăng Quang Thai – Obstetrics, a healthy pregnancy takes place on average about 40 weeks. When babies are born between the 22th and before the 37th week of pregnancy, they are called premature birth. In comparison with full-term babies, premature babies are at risk of health problems such as: respiratory failure, hypothermia, jaundice, underweight, infection, and are likely to have defects such as visual impairment, hearing impairment.  ​

According to the World Health Organization (WHO), premature babies are those born before the 37th week of gestation and are divided into 4 groups: extremely premature (<28 weeks), very premature (28-32 weeks), moderate premature (32 - 34 weeks), late premature (34 - 36 weeks 6 days). ​

Below are the risk factors and signs of threatened preterm birth that pregnant women need to know: ​

► Risk factors for preterm birth: 

♦ Maternal risks: ​

Mothers with chronic pathologies such as: hypertension, diabetes, history of cardiopathy, hepatopathy, nephropathy. Or gestational diseases such as: pre-eclampsia, urinary tract infections, cervicitis, appendicitis, periodontitis…​

Cervical insufficiency case will give preterm birth if intervention is not timely given.​
 
  • Vaginal bleeding during pregnancy ​
  • Maternal malnutrition during pregnancy ​
  • Uterine abnormalities: congenital uterine malformations, uterine fibroids, previous uterine caesarean scar…​

♦ Fetal risk: ​

Cases of preterm birth are likely to be caused by multiple pregnancy, polyhydramnios, IVF, intrauterine growth restriction; placenta previa, intra-amniotic infection … ​

In order to detect the causes of preterm birth, the doctor needs to review full history of obstetric and gynecological pathologies, maternal and fetal risks, the appendages of fetus and combination causes. ​

► Warning signs of the preterm birth: ​ ​
 
  • Pregnant women often feel lower back pain. This can occur constantly or intermittently, but the pain does not subside even if mother changes her lying/sitting posture or massages her back. ​  ​
  • Uterine contractions appear, every 10 minutes or more often. ​ ​
  • Pregnant women with spasms in the lower abdomen or uterine contraction can cause pain. These pains may feel like bloating and may accompany with diarrhea. ​ ​
  • Flu-like symptoms such as nausea, vomiting, or diarrhea. If mothers can't tolerate fluid for more than 8 hours, they must see a doctor. ​
  • Increase intrapelvic or intravaginal pressure. ​ ​
  • Amniotic leakage/Vaginal hypersecretion. ​ ​
  • Vaginal bleeding, including mild bleeding. ​ ​

When there are signs of preterm birth, especially expectant mothers who are in the high risk of preterm birth need to go to the hospital as soon as possible for medical examination. Depending on gestational age at the time of preterm labor, by supportive measures, doctors can prolong the pregnancy, giving the fetus more time inside the uterus to complete the body functions, minimize the negative effects on maternal and fetal health. ​​

► Preventive measures for preterm birth

Pregnant women who have threatened preterm birth need more rest, avoid exertional motions. Simultaneously, they should follow the following tips to limit the risk of preterm birth.
 
  • Comply with prenatal check schedule ​
  • Have proper diet and adequate nutrition ​
  • Avoid stimulants such as smoking, drinking alcohol ​
  • Be cautious when using assisted reproductive technology (ART). ​ ​
  • Treat and control maternal chronic diseases
  • Urinary tract infection and cervicitis screening and treatment are recommended.​
