JOB DESCRIPTION
Clinical pharmacists work in collaboration with other providers to deliver comprehensive medication management that optimizes medication therapy outcomes.
Participate in the analysis, assessment of medication use and in the process of building hospital formulary list.
Monitor medication use in hospital formulary list (including the medication has a narrow therapeutic index, has many serious side effects, antibiotics, drugs need special handling (specializing in Pediatrics, Oncology) by Chief Medical Officer based on the advice of the Drug and Therapeutic Committee (DTC).
Instruct and supervise the use of medicines in the hospital, provide drug information, training in clinical pharmacology
Report about medication use, follow-up, monitoring of adverse drug reactions (ADR) and
Medical consultation, especially for severe cases, patients infected with drug-resistant microorganisms
Scientific research, especially studies related to the issue of safe and effective medicine use, quality improvement issues and improve the effectiveness of clinical activities, and research about optimal medication therapy outcome.
JOB REQUEST
At least 2 years of clinical experience in hospital
Pharmacy practice license
Post graduate certificate is an advantage
Able to communicate English, Reading comprehensive for medical documents
Keep updated international guidelines and able to guide clinical team critical points during practice
Good communication skills
Have a passion in clinical pharmacy and quality improvement that ensure optimal patient’s medication therapy outcome
Use MS Office effectively.
